The CRRT provides clinical consultation, rehabilitation and specialized support to adults with complex disabilities who live in Vancouver Coastal Health region. Clients seen by the team require community-based rehabilitation and/or support in transitioning to the community. This service is designed for clients whose community rehabilitation needs cannot be met by services provided by a Home Health team.
A referral is required in order to access the program.
